I remember when i played before that the 20 DR bonus from this item was only added when having batteries in inventory, and now its not the case (i allways have that bonus) or i am wrong? Does unofficial patch change battery function in some way?

At one point in development the chapeau was indeed consuming batteries, but then they commented it out, yet forgot to change the item description. All the UAP does is change the description to reflect the real state of affairs.

That's nice. Between my Healing Jacket and Flow Spectrometer, I was well sick of trying to keep power cells in supply.

With the jacket's efficiency anyone would be sick. The concept is great, but 4 health points per charge is just insulting.
